What we do: Continuity Clinic Partners is a team of dental professionals helping private practice owners reactivate patients from their own clinical notes.
Scope: We work in strict adherence to HIPAA. We do not diagnose, treat, or refer. Every treatment decision rests with the treating provider.
How it works
1. We start with a conversation. After a brief Zoom call, one of our team members visits your practice in person — to understand your workflow and the kind of treatment that matters most to you.
2. Agreements first. A Business Associate Agreement (BAA) and HIPAA agreement are reviewed and signed before anything else. Non-negotiable.
3. Secure access is established. We set up remote access under strict privacy controls, with defined breach protocols and access limited to what the review requires.
4. You receive your review. In about a week, you'll receive a de-identified list of patients you've seen and diagnosed but who were never scheduled. From there, the choice is yours: we can handle outreach ourselves — following your scheduling preferences and financing options — or we'll walk your front office through an outreach plan, only with your approval.
5. Continue only if it's right for you. If the pilot review earns it, we'd be glad to work with you month to month. No contracts, no pressure.
