About Continuity Clinic Partners
Continuity Clinic Partners was founded on a simple operational premise: many private dental practices do not need more leads before they have a clearer understanding of the production already present within their existing patient base.
In most offices, missed opportunity is not the result of neglect. It is the predictable consequence of a busy clinical environment. Treatment is diagnosed. Patients hesitate. Financial conversations remain unresolved. Recall falls out of rhythm. Follow-up becomes inconsistent. Over time, clinically appropriate care that was once visible becomes buried within the ordinary movement of the practice.
Continuity Clinic Partners exists to help private dental practices identify, organize, and better manage those opportunities.
Our work focuses on chart-level review, incomplete treatment plans, unscheduled diagnosed care, recall visibility, patient reactivation, and case follow-up systems. The objective is not to replace the judgment of the dentist or the function of the practice team. The objective is to provide a disciplined, dental-fluent review of where continuity is being lost between diagnosis, presentation, scheduling, and follow-through.
Founder Background
Continuity Clinic Partners was founded by Justin Howard, whose background includes clinical training at the New York University College of Dentistry, and years of experience in private practice.
That background provides a practical understanding of how dental practices actually function. Not only from the administrative side. Not only from the clinical side. But from the point where the two intersect: the chart, the schedule, the treatment plan, the operatory, the financial conversation, and the patient who leaves without completing care.
This perspective allows Continuity Clinic Partners to approach practice performance at the chart level, where many missed opportunities first become visible.
Our Approach
We do not view production leakage as a generic marketing problem. In many practices, the opportunity has already entered the office. The patient has already been examined. The treatment has already been discussed. The trust relationship already exists.
The breakdown often occurs afterward.
A patient never schedules. A treatment plan remains incomplete. A recall patient becomes inactive. A larger restorative or implant case goes unresolved. A follow-up task is assumed, but not owned.
Continuity Clinic Partners helps practice owners bring structure and visibility to those gaps, so existing patients can be followed up with in a more organized, professional, and clinically appropriate manner.
What We Believe
Every case counts.
Every patient relationship has value.
Every incomplete treatment plan deserves visibility.
Every private practice deserves systems that protect the clinical work, patient trust, and production opportunity it has already earned.
Professional Boundaries
Continuity Clinic Partners does not diagnose treatment, override provider judgment, or pressure patients into care. Clinical decisions remain entirely with the treating dentist.
Our role is to support visibility, organization, and follow-through around existing patient records, existing clinical recommendations, and existing operational opportunities within the practice.
Begin With a Pilot Review
The first step is a focused review designed to determine whether meaningful opportunity already exists within the practice.
